Privacy of Your Health Information Effective 3/1/2003
Dear Suffolk Heart Group Patient:
During your first visit, you completed several forms that included your current and previous personal and family health information. Suffolk Heart Group understands that this information is private and confidential. For this reason, we have several internal policies and procedures to protect your information from unlawful use or disclosure.
- We do not share any protected health information about our patients, former patients or their families to anyone, except as permitted or required by law.
- We ensure that only Suffolk Heart Group employees and agents have access to your personal information. We also ensure that those employees end agents use that information only for appropriate reasons.
- In the event that a physician in this group is involved in research and plans to use patient information, every patient being considered for the study, will be notified and given the option to participate.
- We have many safeguards to protect documents and computer records that include your personal information. With those safeguards, we make sure that your information is not given to unauthorized users.
- If you feel your privacy has been violated please contact the Office Manager immediately.
